Please call 773-878-3750 during office hours to schedule an appointment. You can also request to schedule an appointment through the patient portal. - How can I get prescription refills?
You may call your local pharmacy to refill your prescriptions. - How can we contact the doctor after hours?
After office hours, you can call 773-878-3750. Option 1 for non emergency messages. For urgent messages, press option 3. The operator will connect you to the doctor. - How do I pay online?
